Acupuncture is one of the options people try for neck pain, usually the long-standing, stiff, aching kind with no single injury behind it. The evidence is mixed: some trials show it reduces pain for a while, others show small effects. At Phoenix it is done by our TCM physician, Tan Yi Roe, at Orchard and Tanjong Pagar, and many patients have it alongside neck physiotherapy.

What is acupuncture?
Acupuncture places very fine, single-use needles at specific points on the body, called acupoints. TCM describes these points as lying on channels called meridians, and describes the effect as improving the flow of qi. In physiological terms, the needles stimulate nerves in the skin and muscle and change how the nervous system processes pain signals.

What acupuncture is used for in neck pain
For neck pain, acupuncture is used to bring pain down for a period, which can make it easier to move and exercise, and to ease tension in the neck and shoulder muscles. For people who want to take fewer painkillers, it is one more option to discuss with their doctor.
It is most useful as part of a plan rather than on its own. A stiff, painful neck still needs movement and strengthening, and that is where physiotherapy comes in.
How does acupuncture treatment for neck pain work?
The mechanisms are not fully understood. The main theories are that needling releases the body’s own pain-modulating chemicals, such as endorphins, and changes how the spinal cord and brain process pain signals from the area. TCM explains the same effect as improving the flow of qi through the meridians.
Acupuncture points used for neck pain
Points are chosen for the person, not only the problem. Most are on the neck, shoulders and upper back, but TCM also uses distant points on the hands, legs and feet, and some practitioners use points on the abdomen.
Must be done by a qualified acupuncture practitioner
In Singapore, acupuncture is practiced by TCM practitioners registered with the TCM Practitioners Board, and by doctors approved to practice it. Check that whoever treats you is registered and uses single-use needles.
What happens during an acupuncture appointment for neck pain?
The physician asks about the pain, how long you have had it, what makes it worse, your medical history and the medicines you take, then examines your neck and shoulders.
Very fine needles go in at the chosen points. You may feel a small prick, then a dull ache, tingling or heaviness around the point. It should not feel sharp. The needles usually stay in for 20 to 30 minutes while you rest, and some physicians add gentle heat or electrical stimulation.
Afterwards the physician will tell you what they suggest next and when to check whether it is helping.
How effective is acupuncture for neck pain?
It varies. Some people notice less pain within a few appointments; some notice little change. The research shows benefit for some people, mostly in the short term, and the size of the effect differs between studies. If it is not helping after a few appointments, we stop and look at other options.
Does acupuncture for neck pain come with any risks or side effects?
The common ones are minor: bruising or soreness where a needle went in, and feeling dizzy or light-headed for a short while. The risk of infection is low with single-use needles. Some people have a short flare of their symptoms for a day or so; tell the physician if it lasts longer.
Serious problems are rare. Needling around the neck, upper back and shoulders carries a small risk of puncturing the lung, so difficulty breathing or chest pain after acupuncture is a 995 call.
When neck pain needs a doctor first
Neck pain after a fall or a road accident, numbness or weakness in an arm, new problems with balance or walking, or neck pain with a fever or a severe headache need a doctor the same day, before acupuncture or physiotherapy.

1. It is settling on its own
Most neck pain settles within a few weeks. Stiffness in the morning that eases as you move is usual, and so is pain that flares after a long day at a desk. If the range is coming back and the headaches are less frequent, it is settling the way it should. You do not need us for this, and we would rather say so.
2. It has stopped improving
You reverse the car using the mirrors. You reset your shoulders every twenty minutes and it lasts twenty minutes.
A neck that stops hurting is not the same as a neck that holds up. The deep neck and shoulder blade muscles are trained back, not waited back. That is the gap worth measuring, and it is not measured by how you feel on the day.
