Rib Cage Pain: Heart, Lung or Chest Wall? What to Check First

You reach for the seatbelt and a sharp line of pain runs round your ribs. Now you are taking small breaths and wondering whether this is a muscle or your heart.

If that is the question in your head and the pain is new, the safe answer is to get it checked today. A website cannot tell a muscle from a heart, and neither can a sore spot you can press. Much rib cage pain does turn out to be the chest wall. This page sets out the signs that mean a doctor first, how the common chest wall problems differ once your doctor has cleared you, and what to do when the tests are clear and it still catches.

What causes rib cage pain?

Rib cage pain often comes from the chest wall: a strained muscle between the ribs, irritated rib cartilage, or a bruised or broken rib. It can also come from the heart, lungs, gallbladder, stomach or kidney, or from shingles. Because a sore spot does not rule out the heart, new chest pain is a doctor’s question first.2

Key takeaways

  • Rib cage pain can come from the chest wall or from the heart, lungs or abdomen. New chest pain is a doctor’s question first.
  • A sore spot you can press does not rule out the heart, so new chest pain still gets a doctor’s assessment. In one emergency department study, 12% of people with chest wall tenderness were having a heart attack.
  • Bruised or broken ribs typically heal in about two to six weeks.
  • Most people with costochondritis recover within a few weeks.
  • Once a doctor has cleared you, a physiotherapist tests the rib joints, the upper back and how the ribs move with breathing.

First, the heart and the lungs

  • Call 995 for chest pain that is heavy, tight or crushing, lasts 15 minutes or more, or spreads to the arm, back, neck or jaw, especially with sweating, nausea or faintness. The same goes for difficulty breathing, coughing up more than a few streaks of blood, or a rib injury from a serious accident. The same goes for chest pain or sudden breathlessness with one swollen, painful leg.1,3
  • Go to the emergency department now if breathlessness or chest pain is getting worse after a fall or a blow, or if a rib injury comes with pain in the abdomen or the shoulder, because that can mean damage to the lung, liver or spleen.3
  • See a doctor the same day for any new chest or rib pain with no clear injury behind it, sharp pain when you breathe in, a cough with a fever or colored phlegm, small streaks of blood in your phlegm, pain under the right ribs with fever, yellowing skin or vomiting, pain in the side with fever or pain passing urine, or a band of burning or a blistering rash on one side. In later pregnancy, pain under the right ribs with a headache, vision changes or swelling needs your maternity unit the same day.1
  • See a doctor soon if rib pain has not improved after a few weeks, if a minor knock or a cough caused it and you have osteoporosis, or for pain at night, unexplained weight loss or a history of cancer.3

In one emergency department study, 12% of people with chest wall tenderness were having a heart attack.2 We would rather you went and it turned out to be a muscle.

Already checked, and told it is not your heart or lungs? Then the rest of this page is for you.

Had an operation, a rib fracture or an injection?

After heart or chest surgery, what your ribs and breastbone may do is set by your surgical team, and their rehabilitation program comes first. After a broken rib, including one held with a plate, see rib fracture. After other surgery, see physiotherapy after surgery, and your surgeon’s protocol takes precedence.

If a doctor has given you an injection or nerve block for rib or chest wall pain, whether another one suits you is a question for them. For costochondritis, the evidence for steroid injections is limited.2 Pain easing does not tell you how the ribs and upper back move. More on physiotherapy after an injection.

Which chest wall problem is it?

Chest wall pain is common. It accounts for 20% to 50% of chest pain seen in primary care.2 It usually changes when you press, twist, reach or breathe deeply. That is a clue, not proof, which is why the doctor comes first.

What you noticeWhat it usually points toWho checks it
Constant pain that movement does not change, pressure, or pain with sweating or breathlessnessNot the chest wall, until proven otherwise995, or a doctor the same day
A band of burning or tingling on one side, with or before a rashShinglesA doctor, the same day
Under the ribs, linked to food, fever or passing urine, not to movementAn organ below the ribsA doctor, the same day
Beside the breastbone, several ribs, sore to press, worse with a deep breath or coughCostochondritisA doctor first, then a physiotherapist if it lingers
Between the ribs on one side, sudden, after a twist, a reach or a hard coughAn intercostal muscle strainA physiotherapist
One spot very sore after a fall, a blow or a bad cough, sometimes with a crack felt or heardA bruised or broken rib995 after a serious accident. The emergency department now if breathlessness is getting worse
Lower front ribs, with a click or slip on bending or twistingSlipping rib syndromeA doctor or physiotherapist
At the back beside the spine, on one side, worse twisting or breathing inA rib joint or the mid backA physiotherapist
At the side, below the ribs, after hard sit-ups or twistingAn abdominal muscle strainA physiotherapist

The detail is in costochondritis, intercostal muscle strain, rib fracture, back pain when breathing in, middle back pain on the left and abdominal muscle strain.

Slipping rib syndrome is worth knowing by name. It mainly involves ribs eight to ten, whose cartilage can slip under the rib above. X-rays and CT scans are often normal, and it can come and go for years before anyone names it.4 A clinician tests it by hooking the fingers under the lower ribs and gently lifting them forward.4

How long does rib cage pain last?

Bruised or broken ribs typically heal in about two to six weeks.3 Most people with costochondritis recover within a few weeks.2 A mild intercostal strain usually eases over a few weeks too. Rib pain that has not improved after a few weeks needs another look from your doctor.3

What can you do for sore ribs at home?

For a rib injury your doctor is not worried about: keep breathing normally, take some slow deep breaths, and cough when you need to, holding a pillow against your chest. Use a wrapped cold pack in the first few days. Walk around and move your shoulders rather than lying still, sleep propped up for the first few nights, and leave heavy lifting and sport that hurts for later.3 Shallow breathing feels safer, but the lungs need the movement.

The tests were clear, and it still catches

The thinking goes: they said it is only a muscle, so it will go away.

The ECG was normal and so was the X-ray. Five weeks later it still catches when you reach back for the seatbelt. You breathe off the top of your chest at your desk, you sleep on one side only, and you have told the Thursday badminton group you are busy. Every twinge, you run the checklist again.

Clear tests tell you what it is not. They do not tell you what it is. Has anyone measured how far your upper back turns each way, found which rib joint reproduces the pain, watched how your ribs move when you breathe, and tested what your trunk can take when you reach and lift? If not, you have been told the frightening part is ruled out, and nobody has started on the rest.

What a physiotherapist does with it

Finding the real problem is half the battle. Treating it is the other half.

We do not assess the heart or the lungs. At the Assessment & Treatment, a physiotherapist asks whether a doctor has checked it, and if not, and anything points that way, we stop and send you with a letter. Then come the tests: where it is sore to press, the rib joints at the front and back, how far the upper back turns and bends against the other side, how the ribs move on a deep breath, the neck and shoulder blade, and what reaching and lifting do to it. Scans and injections are a doctor’s decision.

Most of the 45 minutes is treatment: hands-on work for the ribs and upper back, breathing and movement work, and a loading program for the trunk measured up at each review. You leave knowing which stage you are in, what is limiting you, and a written plan.

Every appointment is one to one with your physiotherapist for the full 45 minutes, at five clinics: Orchard, Tampines, Serangoon, Tanjong Pagar and Jurong East. Bring your ECG or X-ray report and your doctor’s letter if you have them.

Questions about rib cage pain

How do I know if rib cage pain is serious?

Call 995 for heavy or crushing chest pain, pain spreading to the arm, neck or jaw, difficulty breathing or coughing up blood. See a doctor the same day for any new rib or chest pain with no clear injury, sharp pain on breathing in, fever, or a rash on one side.

Can rib cage pain be a heart attack?

It can. Pressing on a sore spot does not settle it: in one emergency department study, 12% of people with chest wall tenderness were having a heart attack. New chest pain is a doctor’s question first, and heavy pain or pain with breathlessness or sweating is a 995 call.

What organs are under the right side of the ribs?

The liver and gallbladder sit under the lower right ribs, with the right kidney behind and part of the bowel below. Pain there with fever, yellowing skin, vomiting or pain passing urine is a doctor’s question the same day.

What can I do to relieve pain from bruised ribs?

Keep breathing normally, take slow deep breaths, and cough when needed while holding a pillow against your chest. Use a wrapped cold pack in the first few days, walk around, and sleep propped up at first. Go to the emergency department now if breathlessness or chest pain is getting worse.

What are some good exercises for rib cage mobility?

Once a doctor has cleared you, gentle upper back turns, side bends and slow deep breaths are the usual starting points. What suits you depends on which joint or muscle is involved, and on what your upper back measures against the other side, so a physiotherapist tests that first.

Told it is only a muscle, and it still catches on the seatbelt five weeks on? Get Assessed & Treated, and leave knowing which part of the rib cage is driving it, which stage you are in, and what the next stage asks of it.

Where are you with this right now?

1. It is settling on its own

Most strains and flare-ups settle over weeks with sensible load management and a return to normal movement. Improvement is judged over a month, not over a day, and good days and bad days inside that are usual. If you are doing more this month than last, and sleeping better, it is behaving the way it should. You do not need us for this, and we would rather say so.

2. It has stopped improving

You have adapted without noticing. The other side takes the load, you plan movements you used to make without thinking, and you have quietly dropped things you used to do.

Amazing experience with Phoenix Rehab, was assisted by Mavis. She’s very kind and professional and did thorough check on my wrist. I feel very comfortable during the process. I went in for thermoplastic splint, and everything went smoothly. She was able to address my needs on my left wrist sprain. They were prompt with appointment process and inquiry. Also the clinic is very close to Tanjong Pagar MRT. I highly recommend!

Huge shoutout to Yuna Zhang from Phoenix Rehab for helping in my recovery from golfer’s elbow 🙌 I walked in with persistent elbow pain that was affecting workouts and everyday tasks, and Yuna immediately made me feel confident I was in the right hands. She took the time to properly assess my condition, explain the root cause, and tailor a treatment plan to my specific needs instead of using a one-size-fits-all approach. What I really appreciated was how personalised every session felt. The exercises she gave me were practical, easy to follow, and actually worked. Within a short time, the pain reduced significantly and my strength and mobility improved much faster than I expected. On top of that, the pricing is very reasonable and affordable, which made committing to recovery much easier. Professional, knowledgeable, and genuinely caring — highly recommend Yuna at Phoenix Rehab if you’re dealing with hand-related sports injuries 💪

Pain is the first thing to leave. Movement and strength are the last things trained back, and most people stop before that part is done. That is the gap worth measuring, and it is not measured by how you feel on the day.

The Stalled Recovery Review is three appointments over three weeks with the same therapist, because what the body does under load cannot be read in one visit. The first finds what is limiting it. The next two measure how it responds.

3. You have had surgery, or you have a date

After heart or chest surgery, your surgical team sets what the ribs and breastbone may do. After a rib fracture or other surgery, bring your doctor’s or surgeon’s instructions and we work inside them. See physiotherapy after surgery, or all the recovery programs, hand and physiotherapy.

All of it runs on the same four stages. The Phoenix Recovery Protocol explains what each stage works on and what you have to meet to move up. No referral needed to start.

Next step

Find out which stage you are in

Pain is the first thing to leave. Movement and strength are the last things trained back, and most people stop before that part is done. One Assessment and Treatment is a single 45-minute appointment at $230, nett, no GST. Your therapist finds what is actually limiting you, treats it in the same appointment, and you leave with your current stage and a written plan.
