What the System Gets Wrong
Scans and blood tests are powerful β but they only detect a fraction of what causes pain. The most common causes of persistent musculoskeletal pain are invisible on imaging. That's not a flaw in you. It's a limitation in the tool.
"Your scan is clear. Nothing to worry about."
MRI and X-ray detect bone fractures, disc herniations, and major structural damage well. They do not reliably detect soft tissue dysfunction, nerve sensitisation, tendon pathology, fascial restriction, or the muscular compensation patterns that develop when your body tries to protect an injured area for months.
"The scan didn't find a structural fracture or disc issue."
That's genuinely good news β it narrows down what's causing the problem. But it doesn't mean nothing is wrong. Physiotherapy assessment goes beyond imaging: we assess how you move, where you compensate, which structures are loaded abnormally, and what the nervous system is doing. That's where most persistent pain originates.
"Take anti-inflammatories and rest. It'll settle."
Rest and medication have their place β acutely. For pain that's been present for two weeks or more and is worsening rather than improving, rest alone frequently makes things worse. Tissues weaken, compensation patterns solidify, and the nervous system becomes more sensitised to the pain signal, not less.
"Your body needs structured, directed movement β not avoidance."
Persistent pain that isn't improving with rest needs clinical assessment, not more waiting. The sooner a specialist identifies what's actually driving the pain β whether that's soft tissue, joint mechanics, nerve involvement, or a movement pattern β the sooner a targeted plan can be built to address it.
What Scans Don't Show β But We Find
These are the most common causes of persistent pain that presents with clear imaging. All of them are assessable by hand. All of them are treatable.
Soft Tissue Dysfunction
Muscle guarding, fascial restriction, and trigger points don't appear on scans β but they create significant, localised pain and limit movement. These are among the most common causes of "unexplained" chronic pain.
Sensitised Nervous System
When pain persists, the nervous system can amplify pain signals even when the original injury has healed. The pain is real β neurologically driven. Specific manual therapy and movement retraining directly address this.
Compensation Patterns
When one area hurts, your body unconsciously shifts load elsewhere. Over weeks, those compensating structures become overloaded β and start hurting too. The pain you feel may not be at the original site at all.
Joint Mechanics
Subtle restrictions in how a joint moves β too little glide, too much compression on one surface β won't show on a static scan but cause constant irritation during movement. These respond well to manual therapy.
Tendon & Connective Tissue
Tendinopathy, partial tears, and connective tissue changes are frequently missed or underreported on imaging, particularly in early-to-mid stages. A clinical load test identifies them where imaging cannot.
Nerve Involvement
Nerve irritation, entrapment, and sensitivity produce pain that's diffuse, burning, or radiating β and often confuses both patient and GP. Neural tissue responds to specific mobilisation and desensitisation protocols.
